Approved services may include:
- Residential support
- Respite care
- Homemaker (remove basic or enhanced)

- Supported community connections
- Personal Care
- IHSS Health Maintenance Activities
- Non-Medical Transportation
- Extraordinary Cleaning
This model promotes quality, dignity, and continuity of care at home.

Service Area Note*
JMJ provides direct care and support services for individuals with developmental disabilities. We do not manage the enrollment or eligibility process—that responsibility belongs to Case Management Agencies (CMAs).
CMAs guide families through the application process, create care plans, and determine the appropriate level of support. Once they approve a plan, JMJ delivers the services. These include personal care, respite, day programs, and residential support.
JMJ works with individuals who:
- Live in certain counties and
- Are working with one of the CMAs we’re partnered with.
JMJ currently partners with these CMAs:
- Developmental Pathways – Arapahoe, Douglas, Elbert Counties
- Rocky Mountain Human Services – Denver, Adams Counties
- Jefferson County – Jefferson, Clear Creek Counties
- A&I Avenues – Boulder, Broomfield, Gilpin Counties
- Garfield County – Garfield, Eagle, Pitkin, Summit Counties
